are about to enroll in the
Safinur Study.
To
enroll you need to complete
2 easy steps. In step 1,
please fill and submit the
following forms. If you do
not have all the
information, do not worry.
Just complete the forms to
the best of your knowledge.
By completing these forms
you would have successfully
accomplished the first step.

complete the second step you
will need to have a recent
scan and blood test. Having
done so will facilitate your
enrollment. After completing
these 2 steps your case will
be evaluated and you will be
eligible to obtain Safinur.
The whole process including
step 1, 2 and evaluation
should not take more than 1
week.
